It is a website privacy statement and general professional disclosure. Individual service agreements, informed consent, and any applicable notice of privacy practices are provided separately before services begin.

Professional identity and services

John R. Freeman, M.S., M.A., LCDC
Addiction Counselor • Stress Management Coach • Mind-Body Practitioner

I am a Texas Licensed Chemical Dependency Counselor (Texas LCDC license 15089) and a certified clinical hypnotherapist. Licensed addiction counseling is provided by telehealth to adults physically located in Texas at the time of a session. My services focus on addiction and recovery within the scope of my license, training, and experience.

Stress management coaching, mind-body practice, and provider-guided Safe and Sound Protocol (SSP) services are discussed separately from licensed addiction counseling. Remote SSP services for adults may be available beyond Texas where appropriate. Availability depends on the service, your location, suitability, and applicable requirements. Coaching or SSP participation does not automatically establish a counseling relationship.

Beginning services and informed consent

Visiting this website, sending an email, or leaving a voicemail does not establish a professional relationship. Before beginning services, we discuss your goals, suitability, service scope, fees, payment arrangements, cancellation policies, privacy, communication, and the consent documents appropriate to the service. You may ask questions about an approach, its potential benefits and risks, and available alternatives.

Website information is educational and does not constitute an individualized assessment or treatment recommendation. Results vary; no particular outcome is guaranteed. SSP and mind-body practices are not presented as standalone cures or substitutes for necessary medical or mental health care.

Telehealth, confidentiality, and client records

Telehealth involves technology and may be affected by connection problems or privacy limitations. At the start of counseling, we address your location, a private setting, a backup communication plan, and emergency arrangements.

Confidentiality and its legal limits are explained in the applicable service documents. These limits can include disclosures authorized by you, legally required reporting, certain safety situations, and valid legal requirements. Applicable protections for substance-use treatment records may impose additional requirements. This website statement does not replace those disclosures or authorize access to clinical records.

Information you choose to provide

The current website offers a consultation request form, email, and telephone links for initial inquiries. The form collects your name, email address, phone number, service interest, state or province, country, permission to respond, and your optional permission for a text response. This information is used to respond, discuss availability, and arrange appropriate next steps. Form requests are emailed to john@freemanlcdc.com through the website’s mail delivery system. The form does not intentionally save inquiry content to a separate WordPress submissions database; messages may remain in email systems and their backups. A short-lived, hashed network identifier is used to limit spam requests. Consultation requests do not enroll you in marketing emails. Please keep initial messages brief and avoid sending detailed health information, identification documents, or payment information through ordinary email.

Email and voicemail are not guaranteed to be secure or immediately reviewed. Do not use them for urgent care. We discuss an appropriate method for exchanging sensitive information before services begin.

Website operation, cookies, and service providers

This website uses WordPress and DreamHost hosting. Hosting and website systems may process technical information such as IP addresses, browser information, requested pages, timestamps, and error or security logs to deliver, maintain, and protect the website. WordPress may use cookies for administrative sign-in and other enabled functionality. Your browser settings allow you to manage cookies; restricting them can affect some functions.

Information may be processed by hosting, email, and other service providers needed to operate the website and respond to inquiries, or disclosed when required by applicable law. This page will be updated if forms, analytics, booking, payment, or other data-collection features are introduced. External websites, including Unyte resources and the Unyte Health app, operate under their own privacy policies and terms.

Client services and payments

Client records and payment tools are handled through Carepatron, my electronic medical record system. This website does not collect payment card details. The privacy and consent documents provided when you become a client address clinical records, access, and service-related information; Carepatron also has its own privacy policy and terms.

Privacy questions and requests

Contact me to ask about information you have provided or to request an appropriate correction or deletion. Requests are considered under applicable requirements; professional records and other information may need to be retained for legal, clinical, security, or administrative reasons. No fixed deletion period or unconditional deletion right is promised by this website policy.

Concerns and professional complaints

You may contact me directly about a concern. You are not required to do so before contacting the licensing authority. Complaints about a Texas Licensed Chemical Dependency Counselor may be directed to the Texas Health and Human Services Commission (HHSC). The official LCDC complaint form and instructions (Form 3223) provide the appropriate filing route. For help identifying the LCDC program or its current complaint instructions, contact lcdc@hhs.texas.gov.

Emergencies

This website, email address, and phone number are not emergency services. In the United States, call 911 for an immediate emergency or call or text 988 for suicide and crisis support. Outside the United States, contact local emergency or crisis services.
